– Emergency Contact & Medical Information, Summer (required)
– T-shirt Order Form
Below is the Summer Camp & Workshop Enrollment Form. Once submitted, you will receive a link to complete the Emergency Contact & Medical Information form.
*Note: Campers also attending MLA Summer programs can have their MLA Emergency Contact form used for Mountain SOL. Please see additional information on page 1 of Summer Camp & Workshop Enrollment Form.
